赣南地区2型糖尿病合并非酒精性脂肪肝病的危险因素分析

目的分析赣南地区2型糖尿病（T2DM）患者合并非酒精性脂肪性肝病（NAFLD）的相关因素。方法选择本院内分泌科200例T2DM住院病人，并应用B型超声检查筛选NAFLD病人105例，单纯T2DM患者95例，比较两组之间患者的肝功能、血脂、胰岛素抵抗等相关指标，分析T2DM合并NAFLD的独立危险因素。结果200例T2DM患者中NAFLD合并率为52．5％，其中男性（62例）比女性（43例），男性患病率显著高于女性，T2DM伴NAFLD组患者WHR、TG、HOMA-IR、FINS、ALT、AsT均显著高于单纯T2DM组（Pl〈0．05），而HDL．C显著低于单纯T2DM组（P〈0．05）；胰岛素抵抗、TG与中心性肥胖是其独立危险因素。结论赣南地区2型糖尿病合并非酒精性脂肪肝病患病中男性高于女性，脂代谢紊乱明显，其独立危险因素是胰岛素抵抗、TG和中心性肥胖。

ANALYSIS OF RISK FACTORS IN PATIENTS WITH TYPE 2 DIABETES MELLITUS COMPLICATED WITH NON-A LCOHOLIC FATTY LIVER DISEASE AMONG GANNAN REGION

Objective： To explore risk factors of type 2 diabetes mellitus（T2DM） in patients with Nonalcoholic fatty liver disease （NAFLD） in Gannan Region. Methods： 200 patients with T2DM in the endocrinology department of our hospital were divided into 2 groups according to the results of B type ultrasound, NAFLD group： 105 patients with NAFLD, control group： 95 patients without NAFLD. Liver function, blood fat and insulin resistance of the two groups were compared, the independent risk factors were analyzed. Results： The incidence rate of T2DM patients with NAFLD were 52.5% （male 62, female 43）. The WHR, TG, HOMA-IR, FINS, ALT, AST of T2DM with NAFLD group were significantly higher （P 〈 0.05）, but HDL-C was significantly lower than that of control group （P 〈 0.05）. TG WHR and insulin resistan., ce were the independent risk factors for T2DM complicated NAFLD. Conclusions： The incidence rate of T2DM patients with NAFLD was higher in man than that in women in Gannan. The disorder of lipid metabolism were obvious, the risk factors wereinsulin resistance, TG and central obesity.
