微创治疗颅内血肿40例报道

目的：总结微创清除术在治疗颅内血肿中的应用。方法：40例患，利用CT定位将直径为3mm、适宜长度的颅内血肿碎吸针经电钻置入血肿腔内。经抽吸后，向血肿腔内注入肝素，尿激酶，透明质酸酶液化血肿，经2—5次反复冲洗引流后，彻底将血肿清除。结果：经治疗，21例外伤性颅内血肿患，成活19例，死亡2例，皆因合并有广泛脑挫裂伤，成活率90％；19例高血压性脑出血患，成活18例，1例因中途放弃治疗死亡，成活率95％。结论：颅内血肿微创清除术适应症宽，手术简洁，创伤小，术中生物酶血肿液化剂的应用使我们能在早期或超早期对患实施手术，及早减轻血肿对脑组织的压迫，打破颅内出血后所引发的一系列病理改变。提高了治愈率和生存质量。

Minisurgery Treatment in 40 Cases with Intracranial Hemotoma

Objective:To generalize the application of minisurger in treatment i nt racranial hemotoma.Methods: In 40 cases,useing a diamater 3mm and adequacy lengt h crashing-needle that was drilled into cyslohaematoma with CT locating.To soak old blood and inject heparin,hyaluronidase and urokinase into cyslohaematoma to liquefy the cruor.After repeating the processing 2 to 5 times,the hemotoma is c leared.Results: 21 patiants of traumatic intracranial hematoma,survival in 19 ca ses,death in 2 cases,suvival rate reaches90%.19 patiants of hypertensive inteac erebral hemorehsion,survival in 18 cases,death in 1 cases,suvival rate was95%.C onclusions: Minisurgery in treatment of intracranial hemotoma is wide applicatio n,laconicism and minitraumatic.Biologicalzyme's application make patients be op erated on early stage and hyper-early stage,and mitigate oppression to brain or ganisimion and break out a sevies of patho-trasfrom after blooding,to improve c ure-rate and suvival quality.
