经口咽入路治疗斜坡齿状突型颅底陷入

目的 :探讨经口咽入路手术及围手术期的相关问题及经验总结。方法 :采用经口咽入路切除颅颈交界延髓、颈髓上段腹侧致压物的前路减压。结果 :3例手术顺利 ,术后症状逐渐改善 ,术中硬脊膜破裂1例 ,术后舌肿胀1例 ,口腔菌群失调1例 ,合并颅颈多种畸形后路固定2例。随防5mo至2a,恢复工作和学习各1例 ,1例症状加重再次后路植骨。结论 :斜坡齿状型颅底陷入的治疗唯有切除延髓、颈髓腹侧致压物的前路减压才是有效方法 ,经口咽入路是一种安全有效的方法 ,合并多种畸形颅颈不稳者需植骨固定

On Treatment of Dellen of Slant Dental Basis Cranii By Oropharynx Passage (Attached with Three Cases'Reports)

Objective:To explore operations by the oropharynx passage and relatedissues of the operation span and to sum up the experiences from such operations.Methods:The front part pressure reducing is obtained by operations through the oropharynx passage to cut off the pressing objects in the oblongata and neck marrow on the juncture of craniums and necks.Results:There cases of such operation are successful.The patients are recovered gradually.After the operations,one case had a spinal dura mater damage.One case underwennt the tongue swelling,a case had disorder of the oral cavity fungi.And two cases hadthe operations of back part fixation on the juncture of cranium and neck.BY visiting the patents later,we found three cases restore normal work and study.Conaclusion:The froint part pressure reducing by cutting off the pressing objects in the oblongata and neck marrow on the craniums and necks is the only way to treat the dellne of slant dental basis cranii.Operations through the oropharynx passage is a safer and more effective way.The patient with abnormal cranium and neck should be accompanied by bone transplantation fixation.
