儿童骨盆骨折及其合并伤的治疗

目的探讨儿童骨盆骨折及其合并伤的治疗效果。方法1999年7月～2008年3月的35例儿童骨盆骨折，其中，男20例，女15例，年龄5～16岁，平均11．7岁。骨盆骨折分型按Torode—zieg分类法。Ⅰ型，11例；Ⅱ型，12例．Ⅲ型，8例：Ⅳ型，4例。患者入院后早期明确诊断，及时抗休克，积极处理合并伤，对骨盆骨折予早期复位固定等处理。疗效评价分优、良、差三级。结果35例患者中，死亡2例，33例经6月～7年，平均26．8月的随访。疗效评价，优14例，良18例，差2例，优良率为93．94％（31／33）。结论儿童骨盆骨折应早期明确诊断，骨折早期复位固定，及时治疗合并伤，治疗效果满意。

Diagnosis and Treatment of the Pediatric Pelvic Fracture and its Associated Injuries

Objective To explore the methods for diagnosis and treatment of the pediatric pelvic fracture and its associated injuries. Methods Thirty five children had suffered from pelvic fracture between July 1999 and March 2008. There were 25 boys and 20 girls. The age ranged from 5 to 16 years with an average of 11.7 years. According to Torode - zieg classification, 11 were of grade Ⅰ, 12 were of grade Ⅱ , 8 were of grade Ⅲ and 4 were of grade Ⅳ. All patients were treated with emergency ways, the serious associated injuries were cured at the same time and the pelvic fractures were replaced and fixed as soon as possible. The outcomes of the treatment were graded into the excellent, the good and the poor. Results Thirty three cases were cured, 2 cases were died out of the 35 cases, The 33 cases had been followed up for 6 months to 7 years （median 26.8 months）. The clinical results grade were excellent in 14 cases, good in 18 and poor in 2 out of 33 cases, the rate of both excellent and good was 94.29%（31/33）. Conclusion The early diagnosis and treatment of the pediatric pelvic fracture are very important, the management of the major associated injuries are emergency, the pelvic fracfures should be replaced and fixed as soon as possible, and the outcomes are satisfactory.
