动力髋部螺钉(DHS)治疗老年人股骨转子间骨折

目的探讨应用动力髋部螺钉(DHS)治疗老年人股骨转子间骨折的手术方法及临床效果评价.方法自1999年5月至2004年7月,应用动力髋部螺钉(DHS)治疗老年人股骨转子间骨折21例,男10例,女11例,对得到随访的21例做病例回顾性分析,探讨老年人股骨转子间骨折的特点及DHS内固定手术的技巧.结果全部患者术后随诊7～36个月,平均16个月.疗效评价优6例,良11例,可4例,优良率81%.共有4例发生了术后并发症,其中髋内翻2例,伤口浅表感染1例,泌尿系感染1例.结论老年人股骨转子间骨折,多合并内科疾病及骨质疏松,应用动力髋部螺钉(DHS)对其进行有效的固定,操作简单,固定牢固,可早期进行功能锻炼,有利于骨折愈合及肢体功能恢复.且并发症发生少,优于其他固定方式.手术前后进行骨质疏松及内科疾病的治疗和控制十分重要.

Treatment of Senile Intertrochanteric Fractures with DHS

ObjectiveTo evaluate the curative effects of DHS(dynamic hip screw) in treating senile intertrochanteric fractures and discuss the operative methods.MethodsThe retrospective study covered 21 patients(10 male, 11 female) over 60 years old who had undergone internal fixation with DHS from May 1999 to July 2003. The characteristics of clinical manifestations and curative effects of the senile intertrochanteric fractures were evaluated. And also the operation techniques were assessed.ResultsAll of patients were followed up for a mean time 16 months (from 7 to 36 months). No patients died during hospitalization. 4 cases had postoperative complications, 2 coxa vara, 1 surface incision infection, 1 urinary infection.ConclusionThe senile patients of intertrochanteric fractures often suffer internal diseases and osteoporosis. Applying DHS internal fixation to treat the senile intertrochanteric fractures is an ideal method .The way is more reliable fixation, easy technique, and beneficial to fracture healing and early functional recovery, fewer postoperative complications and it is more effectively than the other way of fixation. The treatment of the accompanied internal diseases and osteoporosis before and after operation is very important.
