保留贲门功能双腔空肠间位移置胃成形术研究

为解决胃癌根治术后达到Ａ级根治程度，有的必须切除３／４以上的胃，以常规手术式重建消化道，由于胃容量过于减少，术后多产生近期和远期的合并症，为了避免术后这一后遗症，因此，建立设计扩大至正常容量而符合正常生理功能的胃成形术式。其中方法为：将近侧空肠褥经横结后提至结肠上区，做成双脓肠襻形成单筒移置于残留贲门部的胃与十二指肠之间，既保留贲门功能又使其形成之代胃并接近正常容量和生理功能，提高了病人的术后生存

THE CLINICAL RESEARCH AND APPLICATION OF DOUBLE-CAVITY JEJUNUM CHEQUERED TRANSPLANTATION OPERATION WITH THE CARDIA FUNCTION KEPT

For degree A,the gastric carcinoma radical o pe ration must sometimes remove three forths of the stomach.Immediate or remote com plication is often caused by the usual operative pattern of rebuilding digestive tract,with the stomach capacity so much reduced.It is necessary to design a new operative pattern to get the capacity enlarged so as to leach that of a normal stomach stomach and make it coincide with the normal physiological function,so a s to avoid the immediate or remote post\|operative sequelae.The close jejunum lo op is lifted up to the upper legion of the colon through the back of the transve rse colon.The jejunum which makes up two cavities to form a single pot is transp lanted to the remaining cordiac part between the stomach and the duodenum,the re maining cardiac part and the substituted stomach almost keep the normal stomach capacity.This method has been applied to three cases.The immediate and remote therapy eff ect is good,The cardiac function is kept,the normal stomach capacity and physiol ogical function are obtained and the post\|operative life quality is improved.The double\|cavity jejunum chequered transplantation stomach\|forming operation keeping the cardiac function suits for the wide\|ranged stomach\|body cancer or the stomach\|body cancer invading the inferior and superior regions.Through the immediate and remote observation and survey,an absolute and radical operation ca n lead to satisfactory therapeutic effects.
