目的 分析癫痫引起精神障碍的特点、治疗效果。方法对1993—2003年在郴州市精神病院住院的癫痫所致精神障碍86例进行回顾性分析。结果精神障碍患者中,原发性癫痫占67%,且多在癫痫发作10余年后出现精神障碍,出现精神障碍前多数有性格的改变。文化低者发病率高,大发作为主。在治疗以抗癫痫与抗精神病药物联合治疗和良好的社会支持,可以取得较满意的治疗效果。结论癫痫病人如反复长期发作可致精神障碍。  相似文献

观察丙戊酸镁缓释片合并阿立哌唑治疗颅脑外伤所致精神障碍伴兴奋激越症状的疗效及安全性。将58例伴兴奋激越症状颅脑外伤所致精神障碍患者随机分为研究组(丙戊酸镁缓释片+阿立哌唑)和对照组(单用阿立哌唑)。两组于治疗前及治疗第1、2、4、8周末采用阳性与阴性症状量表(PANSS)、Cohan Mansfield激越行为量表及临床疗效总评量表(CGI)评估疗效,采用副反应量表评定安全性和药物不良反应。两组PANSS总分在治疗后与治疗前比较,研究组显效率24.1%,有效率86.2%;对照组显效率17.2%,有效率69%,两组间疗效差异有统计学意义(P〈0.05);PANSS兴奋因子分及激越行为量表评分比较,研究组在治疗第2周末与治疗前及对照组比较差异均有统计学意义(P〈0.05),对照组在治疗第4周末与治疗前比较差异才具有统计学意义(P〈0.05);治疗第1、2、4、8周末TESS评分,两组差异无统计学意义(P〉0.05)。丙戊酸镁缓释片能辅助治疗颅脑外伤所致精神障碍患者兴奋激越症状,且起效快、安全性高。  相似文献

探讨颅脑损伤病发精神障碍467例在高压氧治疗后136例患者病情的临床症状改善情况。回顾性分析2004年7月—2013年7月选择颅脑外伤并发精神障碍136例进行高压氧和药物综合治疗,相应对照组302例未行高压氧治疗。对患者临床症状治疗后观察资料进行统计学分析,得出相关结论。本组136例患者在高压氧治疗后治愈62例、显效48例、有效10例,总有效率92.6%,效果明显好于对照组(总有效率78.4%),其中急性期治疗好于慢性期。高压氧对颅脑损伤并发精神障碍有明显的治疗效果,经过临床观察和资料回顾性分析,在同等情况下与单纯药物治疗组有明显的差异。两组总有效率计数的比较采用χ2检验,P0.05为差异有统计学意义。提高外伤性精神障碍的治疗高压氧对病情恢复起主要作用。  相似文献

对抗精神病药物所致药源性精神障碍的发生时间、临床特点、治疗经过及预后所用药物品种、剂量等关系进行初步分析。结果表明:绝大多数患者的药源性精神障碍发生在用药后1-2个月左右,以高效价含氟类药物,长时间大剂量用药,突然停药及躯体合并症者较易发生。  相似文献

探讨常规骨瓣与标准大骨瓣在重型颅脑外伤手术中的应用效果。选择2015年9月-2016年9月进行重型颅脑外伤手术的患者86例作为研究对象,随机将其分为对照组与观察组各43例,对照组采用常规骨瓣开颅术治疗,观察组采用标准大骨瓣开颅术治疗,比较两组患者的手术疗效及术后并发症发生情况。观察组存活率(62.79%)明显高于对照组(60.47%)(P0.05),两组总并发症发生率比较无明显差异(P0.05)。标准大骨瓣开颅术较常规骨瓣治疗重型颅脑外伤患者效果显著,可改善患者预后,提高其存活率。  相似文献

探讨多层螺旋CT用于外伤致肠系膜破裂的临床价值。回顾性分析甘肃省静宁县人民医院从2013年4月到2015年4月急诊治疗的55例外伤致肠系膜破裂患者的临床资料。通过对比分析患者的CT检测结果中主要CT征象,评估多层螺旋CT技术对外伤所致肠系膜破裂的诊断价值。所有患者经多层螺旋CT检测结果显示:52例患者肠系膜密度增高,24例患者肠系膜血肿,38例患者盆腹腔积液,18例患者肠壁增厚,10例患者肠管破裂,16例患者腹腔积气,3例患者合并肝脏损伤。通过对外伤导致的肠系膜破裂患者应用多层螺旋CT检测,其临床应用结果显示具较高的敏感度及特异性,是术前早期确诊及手术方案安排的重要影像学检测方法。  相似文献

目的研究128层螺旋CT三维重建在外伤骨折成像中的临床应用.方法对36例外伤患者进行128层螺旋CT扫描及三维重建.结果 128层螺旋CT对各个部位的骨折均能良好显示.结论 128层螺旋CT三维重建不仅精确地显示病变的立体形态,还能详细了解各解削结构的空间关系,是目前诊断外伤骨折的理想方法.  相似文献

目的探讨颅脑损伤、脑血管病及颅内占位性病变切除术后并发脑性盐耗综合征的原因、诊断及治疗方法 ,以提高颅脑疾病的综合治疗水平.方法回顾性分析54例颅脑损伤、脑血管病及颅内占位性病变切除术后并发脑性盐耗综合征患者的临床资料.结果全部病例均符合CSWS诊断金标准,患者于术后4~10 d血清钠均低于130 mmol/L,24 h尿钠均大于80 mmol/L,血浆渗透压小于270 mmol/L.经治疗,48例患者恢复正常,2例患者入院后病情危重死亡,1例放弃治疗,死于低钠血症,1例死于术后出血性脑梗死,2例为长期植物生存状态.结论脑性盐耗综合征诊断标准是低血钠、高尿钠、低血容量,中重度颅脑外伤术后和鞍区、下丘脑病变患者手术后常并发此症,应该及时给予患者补钠、补液治疗,绝大多数预后良好.  相似文献

目的: 探讨脑弥漫性轴索损伤(DAI)后持续植物状态患者影像学特点并为临床治疗和预后提供影像学建议.方法: 分析30例DAI后持续植物状态病人CT资料,总结、分析其特点及其与临床转归的关系.结果: 本组颅脑外伤后PVS均超过3个月,病人早期昏迷并头颅CT如出现大脑半球多发散在和/或融合的高密度出血灶,主要累及额叶、颞叶、基底节区者,则预后不良.结论: 脑弥漫性轴索损伤(DAI)后持续植物状态患者影像学特点可给临床治疗和预后提供较可靠的影像学建议,累及额叶、颞叶、基底节区者,预后不良.  相似文献

目的:对颅脑外伤进行早期骨修补手术治疗的临床疗效进行观察和分析.方法:选取我院颅脑外伤患者56例,分为观察组和实验组各28例.观察组行传统的开颅手术以清除血肿,实验组在此基础上对血肿部位行早期骨修补术治疗,观察两纽患者治愈情况及并发症发生率.结果:观察组有效率为71.43%,并发症发生率65.00%而实验组有效率为96.43%,并发症发生率只3.70%,统计学分析,P0.05.结论:对颅脑外伤患者开颅清除血肿基础上进行早期骨修补手术的治疗方法,有较高的治愈率,且并发症发生率极低.  相似文献

The discovery of the prolific Ordovician Red River reservoirs in 1995 in southeastern Saskatchewan was the catalyst for extensive exploration activity which resulted in the discovery of more than 15 new Red River pools. The best yields of Red River production to date have been from dolomite reservoirs. Understanding the processes of dolomitization is, therefore, crucial for the prediction of the connectivity, spatial distribution and heterogeneity of dolomite reservoirs.The Red River reservoirs in the Midale area consist of 3~4 thin dolomitized zones, with a total thickness of about 20 m, which occur at the top of the Yeoman Formation. Two types of replacement dolomite were recognized in the Red River reservoir: dolomitized burrow infills and dolomitized host matrix. The spatial distribution of dolomite suggests that burrowing organisms played an important role in facilitating the fluid flow in the backfilled sediments. This resulted in penecontemporaneous dolomitization of burrow infills by normal seawater. The dolomite in the host matrix is interpreted as having occurred at shallow burial by evaporitic seawater during precipitation of Lake Almar anhydrite that immediately overlies the Yeoman Formation. However, the low δ18O values of dolomited burrow infills (-5.9‰~ -7.8‰, PDB) and matrix dolomites (-6.6‰~ -8.1‰, avg. -7.4‰ PDB) compared to the estimated values for the late Ordovician marine dolomite could be attributed to modification and alteration of dolomite at higher temperatures during deeper burial, which could also be responsible for its 87Sr/86Sr ratios (0.7084~0.7088) that are higher than suggested for the late Ordovician seawaters (0.7078~0.7080). The trace amounts of saddle dolomite cement in the Red River carbonates are probably related to "cannibalization" of earlier replacement dolomite during the chemical compaction.  相似文献

本文叙述了对海南岛及其毗邻大陆边缘白垩纪到第四纪地层岩石进行古地磁研究的全部工作过程。通过分析岩石中剩余磁矢量的磁偏角及磁倾角的变化,提出海南岛白垩纪以来经历的构造演化模式如下:早期伴随顺时针旋转而向南迁移,后期伴随逆时针转动并向北运移。联系该地区及邻区的地质、地球物理资料,对海南岛上述的构造地体运动提出以下认识:北部湾内早期有一拉张作用,主要是该作用使湾内地壳显著伸长减薄,形成北部湾盆地。从而导致了海南岛的早期构造运动,而海南岛后期的构造运动则主要是受南海海底扩张的影响。海南地体运动规律的阐明对于了解北部湾油气盆地的形成演化有重要的理论和实际意义。  相似文献

The explosive growth of the Internet and database applications has driven database to be more scalable and available, and able to support on-line scaling without interrupting service. To support more client's queries without downtime and degrading the response time, more nodes have to be scaled up while the database is running. This paper presents the overview of scalable and available database that satisfies the above characteristics. And we propose a novel on-line scaling method. Our method improves the existing on-line scaling method for fast response time and higher throughputs. Our proposed method reduces unnecessary network use, i.e. , we decrease the number of data copy by reusing the backup data. Also, our on-line scaling operation can be processed parallel by selecting adequate nodes as new node. Our performance study shows that our method results in significant reduction in data copy time.  相似文献

R-Tree is a good structure for spatial searching. But in this indexing structure,either the sequence of nodes in the same level or sequence of traveling these nodes when queries are made is random. Since the possibility that the object appears in different MBR which have the same parents node is different, if we make the subnode who has the most possibility be traveled first, the time cost will be decreased in most of the cases. In some case, the possibility of a point belong to a rectangle will shows direct proportion with the size of the rectangle. But this conclusion is based on an assumption that the objects are symmetrically distributing in the area and this assumption is not always coming into existence. Now we found a more direct parameter to scale the possibility and made a little change on the structure of R-tree, to increase the possibility of founding the satisfying answer in the front sub trees. The geographic information service is enabled by the advancements in general Web service technology and the focused efforts of the OGC in defining XML-based Web GIS service. Based on these models, this paper addresses the issue of services chaining,the process of combining or pipelining results from several interoperable GIS Web Services to create a customized solution. This paper presents a mediated chaining architecture in which a specific service takes responsibility for performing the process that describes a service chain. We designed the Spatial Information Process Language (SIPL) for dynamic modeling and describing the service chain, also a prototype of the Spatial Information Process Execution Engine (SIPEE) is implemented for executing processes written in SIPL. Discussion of measures to improve the functionality and performance of such system will be included.  相似文献

Advances in wireless technologies and positioning technologies and spread of wireless devices, an interest in LBS (Location Based Service) is arising. To provide location based service, tracking data should have been stored in moving object database management system (called MODBMS) with proper policies and managed efficiently. So the methods which acquire the location information at regular time intervals then, store and manage have been studied. In this paper, we suggest tracking data management techniques using topology that is corresponding to the moving path of moving object. In our techniques, we update the MODBMS when moving object arrived at a street intersection or a curved road which is represented as the node in topology and predict the location at past and future with attribute of topology and linear function. In this technique, location data that are corresponding to the node in topology are stored, thus reduce the number of update and amount of data. Also in case predicting the location,because topology are used as well as existing location information, accuracy for prediction is increased than applying linear function or spline function.  相似文献
